The Influence of Social Support, Resilience and Depressive Mood on Death Attitude of Elderly Patients on Hemodialysis

Popis: | 107 Background: Elderly hemodialysis patients have a high level of depressed mood. However, concerning the correlations among depressed mood, attitude towards death, and attitude towards euthanasia, in-depth research exploring the mediating effect of social support and resilience has not yet been conducted. By exploring the positive personality traits and social support forces, it will help promote a positive attitude towards death and prepare for a good death. Objective: To explore the general situation of the elderly hemodialysis patients’ social support, resilience, depressed mood, and attitude towards death, as well as the relevance among the important variables. We conducted in-depth research exploring the mediating effect of the research participants’ social support and resilience on depressed mood and attitude towards death. Method: A cross-sectional quantitative research design was adopted, with people aged 65 years and above from the hemodialysis centers of two teaching hospitals in Hsinchu as research participants, 209 elders in total. The structured questionnaire was adopted for data collection. The questionnaire content included: basic demographic data, social support scale, resilience scale, depression scale, attitude towards death scale, and euthanasia scale. The SPSS20.0 and LISREL9.30 statistical software were used for data analysis. Results: (1) In terms of social support, “information support” had the highest score. The related factors affecting social support show that the higher the number of years after undergoing hemodialysis, the higher the number of comorbidity and disease symptoms, indicating the soundness of the social support system for the family members and medical and nursing personnel; (2) In terms of resilience, “a meaningful life” had the highest score, while “accepting the loneliness of existence” had the lowest score. The related factors affecting resilience show that people who were young-old, with higher education level; higher self-perceived health; and less hemodialysis symptoms, had better resilience; (3) In terms of depressed mood, “without depressed mood” comprised the majority (accounting for 58.9%). In terms of impact on depressed mood, people who were single, elderly, with self-perceived poor health and with more hemodialysis symptoms were more depressed; (4) In terms of attitude towards death, accept death had the highest score, while fear of death had the lowest score. In terms of impact on a positive attitude towards death, people who were young-old and never had a shock were better able to accept death; in terms of impact on a negative attitude towards death, people with lower education level more feared for death; those with hemodialysis symptoms were prone to escape death and had an escape-oriented attitude towards death; (5) 57.4% of the subjects agreed to euthanasia, and those with hemodialysis symptoms were significantly associated; (6) Verification of the mediating effect in the model: Social support produced a complete mediating effect on depressed mood and a positive attitude towards death; (7) Social support produced a partial mediating effect on depressed mood and a negative attitude towards death. Conclusions and Recommendations: The elderly hemodialysis patients in this study were in a state of lower resilience and higher depressed mood. In addition to providing hemodialysis related care and alleviating troubling symptoms, the elderly’s psychological resilience and social support must be strengthened to allow current treatment to become meaningful for living and to facilitate the development of a positive attitude towards death. The research results are expected to be provided to hemodialysis department nursing care personnel to plan resilience intervention plans or build a social support environment, thereby serving as a reference for enhancing the quality of care for hemodialysis patients. |
