An industrial disaster usually takes place with the loss of life and/or property. The most serious crisis is that such disaster may cause a company unable to continue to operate. Therefore, the prevention of industrial disaster recurrence is also regarded as an important safety indicator. To prevent the recurrence of industrial disasters, it is very important to investigate the actual accidents correctly. Through the accident investigation, find out the actual causes of the accident and improve it to reduce the chance of recurrence. In particular, for the chemical industry batch process with high level of disaster severity, it is more eager to effectively identify the cause of the accident for reducing the chance of disaster recurrence. In this study, the Human Factors Analysis and Classification System (HFACS) in conjunction with the Level of Analysis (AHP) approach is used to complement the gaps in the traditional survey of traditional accidents in the assessment of human factors. It is the ultimate goal and contribution of this study to correctly and systematically identify the main causes of human factor errors in industrial disasters.
