Action research has been conducted for more than two and a half years, at a senior care center in southern Taiwan for a spiritual care model construction among the aging population. The participants of the study include nurses, caregivers, social workers, counselors, managers, administrators, volunteers, and fifteen long term care residents in the center. Eight seniors have been provided with in-depth spiritual care, and the process and outcome were documented and analyzed to form the basis for the research. This text research starts with the initial level of description and analysis. First, it describes the action research process of the spiritual care, and the characteristics of the elderly participants, the individualized services, and the interpretation of their spiritual lives. It then reports caregivers’ interpretations of the aging phenomenon of the elders under care, and the issues related to the meanings of dying and spiritual care. Finally, it discusses caregivers’ own spiritual needs and the needs for spiritual care upon their aging. Topics under investigation include the interpretation of various issues such as death and dying, spiritual/soul phenomenon, and religious beliefs. Issues in relation to institutional care, organizational management, inter-professional cooperation and community interaction are also addressed. The theoretical perspectives on spiritual awareness and development have contributed to a four-stage process for spiritual care capacity for the caregivers: (1) contemplating, (2) initial testing, (3) developing, and (4) the synergy of spiritual caring capacity and own spiritual transformation. Components of a spiritual care model for the elderly people in institutions are thus identified. First, it is focused on spiritual transformation(including encountering spiritual related issues, initial spiritual awareness, reflection, re-learning, spiritual growth and deepening). Second, it is centered on the capacity building for the caregivers(including basic care capacity and career development). Third, it is tied to mediator’s development of care ability(including care management and career development. Forth, it focuses on spiritual care activities to be provided(including basic care needs, spiritual needs, companionship needs. And finally, it is about the interaction of care response(physical and mental symptoms, dying reaction, spiritual suffering)and the use of care mechanism(including facilities and equipment in the hospital, multi-faceted care mechanism, and the care system adjustment facilitated by the intermediary. The researcher’s personal reflections and suggestions for further research are also included.
