A study of the Business model of Post-Acute Care-In case of a hospital in southern Taiwan

Purposes In October 2013, the Ministry of Health and Welfare announced the “People''s Health Insurance Improvement of Post-acute care Quality Pilot Program (PAC)” and started the pilot project in regional hospitals and regional hospital beds in 2014. In 2017, we will expand the scope of implementation and ensure the continuity of more patient care by expanding the scope of services. Although there are hospitals involved in the scheme of post-acute care, there is no complete business model for the operation of PAC. We conduct a case study to explore the business mode of post-acute care then to provide some solutions and suggestions for PAC services model. Methods This study used the case study method proposed by Yin (1994) to design the research process. Through qualitative interviews, the hospitals describe the current status of the post-acute care operations and the problems they were faced, and discover the factors behind them and provide solutions to the problems. The interviews are based on nine elements of the business model proposed by Osterwalder (2010) and are classified according to four aspects of the business model proposed by Al-Debei and Avison (2010). Result In order to respond to the aging population and disability, the government actively promotes PAC and vertically integrates care services to provide continuity of patient care. However, many hospitals face certain financial risks in order to establish a PAC. The study found that the case hospital''s business model for PAC: Value Proposition is based on the treatment of strokes, and the interdisciplinary PAC team provides relevant rehabilitation treatments. Value Configuration, telecom tracking and discharge service staff assistance, and related discharge services and related community education, to establish a relationship with patients. Value Architecture, through cooperation with the system hospital and the PAC team hospital, to receive eligible patients, and participate in relevant activities of the related stroke associations from time to time. Value Finance, due to the impact of the health insurance system, its income is mainly for health insurance. In terms of cost, compared with other departments, its labor cost accounts for 80% of the income. Because the income is limited, the hospital faces financial risks. Conclusion and Discussion Based on the current case hospital business model, plus some areas for improvement and development. First of all, in terms of manpower, it is found that the importance of a Case Manager in the PAC model is an important bridge between the hospital and the patient. At the same time, it can also share the clinical staff''s services related to the discharge of patients, so it is more prominent in the model. The importance of a steward. Value Finance, at the customer level, expands the room for development by increasing fragility fractures and day care. In cooperation with home agencies, long-term institutions and other related institutions, the continuity of care services has been established. The cost rising, key resources control, and unnecessary expenditures that recommended to set up a PAC service related group, setup a single window, and establish relevant medical treatment procedures. Additionally patients can be transferred to the upstream hospital and internal relevant personnel operate the administrative process and reduce the related inspection costs and bed use by establishing relevant processes and systems. It is hoped that relevant information technology can be used to assist, regardless of the patient''s control and clinical application, through the use of technology to achieve savings in manpower and care costs management. Although it is possible to operate in an innovative way through BM in line with the changes in the industrial environment. The PAC is to continue to operate, the government''s health care payment and related regulations still have room for adjustment, and the medical industry will be changed through government adjustment.
