A Study of Related Factors in Home Care Nurses of Practicing Stress:An example of Taichung city

Taiwan has entered an aging society, which leads to the increased demands of long-term care and investment in home-care nurses. This research aims to explore the practicing pressure and relevant factors of home-care nurses. Home-care nurses in Taichung city were investigated. Data were collected by means of structured questionnaire and the contents involved include: personal attribute variables, practicing motivation, practicing patterns, main service contents of home care, work hours, nursing business, practicing difficulties as well as practicing pressures. Two hundred and thirteen copies of questionnaires were sent out, of which 183 valid copies were collected, with a recovery rate of 86%, which were filed for analysis with SPSS 19.0. The research results were as follows: home-care nurses interviewed were aged 41.1±0.58 on average with a job seniority of 5.99±5.24. Of the surveyed, 62.84% of them were from independent-type home-care center and 80.7% of them were full-time workers with average daily work hours of 7.31±3.04. They were responsible for 58.06±32.06 persons and carried out nursing supervision for 72.12±34.9 persons every month on average. The service contents included: physical assessment, nursing instruction, replacing nasogastric tube, catheter, autogenous cutting; the wound and making-oral nursing, health education of diseases, health consultancy. In addition to the foregoing services, they are also engaging in: 94% of medical equipment and accessories instruction, 90% of cross-team referral, 87% of social resources referral, 83% of medical equipment and accessories rental consulting as well as 70% contact of return for outpatient services. The difficulties in practice they are aware of include: the order is as follows according to the score: administrative management (79.25), professional service (61.96), safety management (60.50), human and material management (57.00); as for the nursing pressure, the order is the same as the difficulties: administrative management (76.75), safety management (54.13), human and material management (53.04) and professional service (51.96). The research further explored relevant influencing factors of practicing pressure in home-care nurses from the perspective of stepwise regression and the canonical regression formula obtained is as follows: Y(total score of pressure)= 0.788(total score of difficulty)-0.116( with on-the-job training)-0.152(age)-0.114(education hours)-0.099(full-time) The results showed that the main factors influencing the practicing pressure of home nurses were not only consist of being self-aware of the difficulty of the job; as for those who never took on-the-job education or took fewer hours of on-the-job education and worked part-time, the younger they were, the higher the pressure would be. In view of this result, it was suggest that competent authorities should streamline the administrative paperwork workload of home-care nurses and healthcare insurance application procedures, reduce supervision and appraisal processes and assessment operation as well as improve the safety management of home visit; in addition to alleviating work difficulties, it is better to increase the occurrence of workshop, encourage home-care nurses to participate in educational training, encourage full-time nurses to reduce practicing pressures and encourage young home-care nurses to dedicate to their work and retain them in their posts as well as improve care functions of home-care nurses.
