Outcome Evaluation on Ventilator Therapy in Respiratory Failure Patient

The medical expenditure for ventilator-dependent patients due to respiratory failure was 16.1 billion in 2012,accounting for 22.7% of total expenditure for major illness, which data from National Health Insurance Administration Ministry of Health and Welfare in Taiwan. Despite previous effort to improve care quality for ventilator-dependent patients and to decrease medical cost, the huge medical expenditure was still concerned by our government. In this study, we conducted a retrospective secondary data analysis from National Health Insurance Database in 2005, which comprised of approximately 1,000,000 randomly sampled subjects. Study subjects were diagnosed as respiratory failure (International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9-CM) disposition codes 518.81–518.89) were enrolled in this study. The primary outcome was the weaning rate of different strategy for patients with respiratory failure(oral endotracheal tube intubation or NIPPV). Besides, this study also analyzed length of hospital stay, medical cost and mortality rate in two groups. Other factors that influence the weaning outcome were also discussed, such as gender, aged and so on. This study used SPSS software(Version 20.0,SPSS, Inc., Chicago, IL, USA) for data analysis. Using descriptive statistics drew the picture of study subjects’ characteristics. This study employs independent Student's t-test, multivariate regression analysis and cox-regression to detect the difference between two groups. Research result found that medical costs of noninvasive positive pressure ventilator (27,739,892.11,11,285,085.10,15,003,931.06) below the invasive ventilator (61,925,309,64,077,313.41,58,869,947.70). The number of days hospitalized of noninvasive positive pressure ventilator (39.67,29.00,35.67) below the invasive ventilator (55.73,148.04,252.26).
