The Study on Children and Adult Systemic Lupus Erythematosus with the Risk of End-stage Renal Disease and Medical Utilization

Objective: Systemic Lupus Erythematosus (SLE) is the most serious violations of the kidneys. There will be usually formed End Stage Renal Disease (ESRD) in a short time if the nephritis haven’t been controlled well enough. Thus, the study is to investigate the situation of children and adults suffering from Systemic Lupus Erythematosus for End Stage Renal Disease and the risk of medical utilization. Method: The design of this study is retrospective study with secondary data analysis. The data are obtained from Longitudianl Health Insurance Database 2000 and 2005 assembled by National Health Insurance Research Database. Systemic Lupus Erythematosus patient''s sample for the ICD-9-CM 710.0 patients; Gender and age quadruple pairing were adopted in control group. Use Cox models to analysis the risk of End Stage Renal Disease and T test to explore the differences in medical utilization. Results: In this study, there are 4666 samples of SLE patients. Compare to the Non-SLE patients (control group), the risk of SLE patients suffering from ESRD is higher about 3.74 times, the adult patients is higher about 3.89 times and the children patients is higher about 2.31 times. There are three significant differences in statistics. And compare to the adult SLE patients, the risk of suffering from ESRD is about 0.6 times higher to the children SLE patients. In pharmacotherapy way, those SLE patients who have use immunoregulatory drugs will suffer from ESRD than those who do not have use immunoregulatory drugs for 2.41 times. In this study, using T test to check the medical utilization difference between adult SLE patients and children SLE patients; according to average medical utilization, the number of outpatient, hospitalization, outpatient fees and hospital costs have significant differences, but hospitalized. In the test results both adult and children patients have significant differences in using medical utilization. Conclusions and Suggestions: SLE does increase the risk of ESRD and the drugs treatment does increase the risk of ESRD risk. In medical utilization way, the children patients are higher than the adult patients. Therefore, the proposed health authorities and medical staff should attach great importance to children SLE patients, in order to avoid the limited medical resources to spend more of the cost and delay patient health status situation recession occurs.
