Risk assessment of dietary intake of aluminum-rich foods in Taiwan

Background: Aluminum is the most abundant metal element (about 8%) in the earth crust. The ingestion is the primary exposure route to the body. It will cause adverse effects to bone, blood or nerve system from long-term exposure. WHO suggested a provisional tolerable weekly intake (PTWI) of 2 mg/kg bw-wk for aluminum in 2012. The Taiwan dietary exposure model for aluminum remains to be established. Object: In this study, we considered the processing characteristics of aluminum-rich foods, focusing on those foods that have aluminum food additives and high natural background of aluminum. Accroding to Codex guideline, exposure scenarios of general population and sussecptible groups were proposed to estimate healh risks so that their difference could be compared with other countries. Method: We evaluated the dietary exposure of 19 groups of aluminum-rich foods for 19 narrowed shortened list of core foods in Taiwan. We collected on previous analysis results of 12 groups and searched the remaining groups for concentration data from literatures. As analytical results was below the limit of detection (LOD), it was set to 1/2 LOD to calculate the mean upper-bound concentration of the group of core foods. Four databases of Nutrition and Health Survey in Taiwan (NAHSIT) were combined to cover an entire range of age. Aluminum-rich foods were then identified and labeled with systematic codes. Using the Observed individual mean (OIM) method, two estimates of mean and 95th percentile were calculated for each exposure scenario by the Consumption Rate (CR) computer module of the Taiwan Dietary Exposure Evaluation Model (TDEEM) developed in this study.
