Due to the uncertainty of the healthy insurance payment system, and medical market intense competition, hospital survival, and reduction of operating costs, outsourcing is one of the important strategies in non-core business. The majority of hospitals have taken the outsourcing strategy, including labor-intensive, low-skill levels, such as cleaning, security, gardening, parking, and so on. Hospital Engineering System provides the medical supply and the demand of power sources, which has continual, particular and uninterrupted characteristics. Therefore, personnel cultivation, professional skills, maintenance of resume information, and a continuation of the experience are related to the quality of the system supply. Furthermore, it definitely affects the medical demand and supply. Whether the outsourcing strategies of Hospital Engineering System can be implemented, how to reduce the operating costs is the focus of the research. Through the study, we can find that the Hospital Engineering System adopting the outsourcing strategies accounts for more than 50%. In the part of the core resource, among the bed-fewer hospitals, non-teaching hospitals evaluation-qualified hospitals, the core resource of the engineering system is getting less, while the dependence on external resources is getting more. Hospital electricity and water supply and drainage systems are the main resource supply for the other systems. Owing to the various functions and instruments or equipments, each hospital has the unique and specific water and electricity supply systems. The hospital engineering department should regard the development of the water and electricity systems as the core business. In the part of. The pre-cost of the transaction cost theory, and the standard of the outsourcing partners’ selection, this study shows that it contains two primary aspects, including the ideal aspect and the practical aspect. The priority of the ideal aspect is technology, license, price, qualifications and scale. The priority of the practical aspect is technology, price, license, qualifications and scale. The priority of the best-evaluation hospitals selecting the cooperative partners is technology, qualifications, price, license and scale. In the part of The post-cost of the transaction cost theory, the organization and outsourcing partners are supposed to be on a regular or casual basis involved in the mechanism of communication and negotiation. In addition, concerning system repair, maintenance, review, analysis, the outsourcing companies are bound to pay more attention and to achieve the requirements. The agent security of Agency theory, the outsourcing partners who act for the execution of hospital business ought to take the protective measures during the operation, in order to prevent the ignorance in the Hospital Engineering System. In case, it may increase not only the agency costs but also the costs of dealing with the hazards or emergencies.
