The effect of interferential current treatment over acupuncture points on low back pain with sciatica

Background: Interferential current (IFC) is a popular modality used by clinician for musculoskeletal pain control, including low back pain (LBP). However, the evidence is not strong enough to support the effect of IFC for back pain control till now. In particular, the effective stimulation sites for treatment of LBP have not been clearly determined. Purposes: The aims of this study were to assess the effect of interferential current on low back pain with sciatica, and to compare the effect of interferential current over different stimulating sites for low back pain with sciatica. Study design: double-blinded randomized controlled trial. Materials and Methods: Fifty six subjects, having a diagnosis of low back pain with sciatica and meeting the selection criteria of this study were recruited. They were randomly assigned into either three active IFC groups stimulation over acupuncture points (n=16), spinal nerve roots (n=16), painful area (n=16) or placebo group (n=16). The carrier frequency 4000Hz, Amplitude-modulated frequency (AMF) of 80-100 Hz and Current intensity 30% above pain threshold were set in three active groups except zero intensity setting in placebo group. Four cup electrodes were used to stimulate for 15 minutes per session, and a total of 20sessions during 4-weeks intervention. Main outcomes including pain condition (intensity and distribution) and functional performance (bend to touch ground、repeated trunk flexion、repeated sit to stand、fifty foot walk) were assessed at pre-, post-10, and 20-time treatment, and global perceived rating were assessed at post-10, and 20-time treatment. Pain intensity before and after each treatment were also recorded by visual analog scale. Two-way (group x time) repeated measures ANOVA and Bonferroni post hoc test were used in data analysis. The significant level was set at 0.05. Results: The basic characteristics were not significantly different among four groups. A significant interaction effect was found in the reduction of pain intensity (p=0.001).
