Research on Applying the Continuity Care Document to Generate a Medical Record with Entry Level

Transferring patient information between medical care sites is necessary to deliver better patient care and to reduce medical cost. So developing of electronic medical records is an important trend for the world. In the actually status, the HL7 Clinical Document Architecture (CDA) is currently most widely used international standards for electronic medical records. However, it is too difficult to implement CDA due to the complexity of information model and enormous data types on entry level. This study combines the advantages of two standards. One is CDA that is flexibility and powerful architecture and the other is Continuity of Care Document (CCD) that provides the methodology to implement body and entry level. It also offers a large step towards the goal of interoperability among EHRs. Based on the two standards, we developed a system to generate clinical document with entry level. This system allows users to design their standard-based clinical document format more easily and quickly, even without the knowledge base of CDA or CCD.
