The Analysis on the Health Care Expenditure in the Last Two Years of Life

Our empirical tests are based on data from the Longitudinal Health Insurance Database 2005(LHID2005) of the National Health Insurance Research Database (NHIRD). Quantile regression was applied to analyze health care expenditure in the last two years of life from 2005 to 2009 in Taiwan. This paper derives the demand for health care expenditures in the last two years of life from a model that accounts for age, gender differences, age and gender interactions, time to death (quarter) and years. The findings show that health care expenditures will grow with age, or the impact on health care expenditures is relatively small with age in the high level population of health care expenditure. Health care expenditures of the population over the age of 65 will decrease with age. The impact on time to death, the higher level of health care expenditures, before the death in the first quarter (Q1) will be smaller. On the time point of view, health care expenditures surge from the first quarter (Q1) to the second quarter (Q2). This is the same circumstance on each quantile point of health care expenditures. The population over the age of 65 did not increase in health care expenditures, but it still fluctuated a lot in the last quarter of life. Therefore, the impact of age on health care expenditures is limited and is just one of the factors affecting health care expenditures. Medical resource utilization before death is the main reason for the growth of overall health care expenditures.
