The Prevalence of Otitis Media in Children of Taiwan—A National Health Insurance Research Database Analysis

Otitis media is a common disease in childhood、which antibacterial agents are usually prescribed for treatment. The disease becomes a big concern for the patients of children because the symptom of high fever is easily occurred in children at acute stage of otitis media、and because of the possibility of hearing loss due to otitis media with effusion. The large prospective studies in the United States revealed that at least 90% of children have one or more bouts of otitis media by the age of 2 years、but studies in Taiwan revealed lower prevalence of otitis media in pre-school and primary school age children、due to differences in utilizing the methodology、of regions、or of ethnicities. In this study、we try to estimate the prevalence of otitis media in children of Taiwan by using the National Health Insurance Research Database (NHIRD)、instead of inspecting each child in kindergartens or primary schools. We obtained twelve cohort datasets form the NHI research database. Each cohort included 50,000 people randomly sampled form people who had ever been insured under the NHI since 1995. We further retrieve all children who were 10 years old in 2006 and 2007. Then、we analyze their medical records of otitis media since the birth to 10 years of age. The prevalence of otitis media in each year were between 0.79% to 14.61% in the period of 1 to 10 years of age、the higher prevalence occurred at 3 to 6 years of age and the highest was at 5 year of age. The accumulative prevalence otitis media at age 2、5、and 10 were 7.95%、35.24% and 50.24%. Otitis-related surgical procedures were performed on 1.97% of children who have otitis media in this study、and the most commonly performed procedures was myringotomy with ventilation tube insertion. About half of the children in Taiwan have one or more bouts of otitis media by the age of 10 years、50% of these children only got otitis media in one year before 10 year of age. The prevalence of otitis media in children of Taiwan was lower and the peak age of incidence was higher to the study results in Western countries、but the ratio of otitis-related surgery was similar.
