The Relationship of Nutritional status and Physical function, Hospital Admission Rate, Longer hospital Stay, Mortality among Elderly in Long-Term Care Facilities

The rate of 20.7% of the undernourished elders in internal long term care facilities reflects the general phenomenon of the nutrition problems of the elders in the long term care facilities. To improve the ability of mastering nutritional status and the concerns about the nutrition issues toward elders of the nursing staff in long term care facilities, this study is aimed to explore the relationship of nutritional status and physical function, hospital admission rate, longer hospital stay, mortality among elderly in long-term care facilities and then to promote elder care quality. Using the retrospective study design and the convenience sampling, this study included 174 elders, who were over 65 years old and lived in legally registered elder care facilities in Beitou District of Taipei City. For the available participants, they were living in the facilitity for at least one year that counts from the day of data collecting, including the dead persons. The researcher of this study collected the data of albumin, hemoglobin, total cholesterol, body mass index, calf circumference, barthel index, hospital admission rate, longer hospital stay and mortality causes from the elders’ physical examination reports. All the data are analyzed by the SPSS 17.0, and the statistical techniques includes frequencies, percent, mean, Std. Deviation, McNemar’s test, independent sample T test, one way ANOVA, logistic regression, generalized linear models, chi-square, Pearson’s product-moment correlation, Goodman-Kruskal and Tau Kaplan-Meier. The result of this study showed that most elders in the elder care facilities have good nutritional status except the part of calf circumference, which proves the obvious relationship between physical function and calf circumference. Other results also showed that several factors affected the health status, including calf circumference, albumin, total cholesterol, and the case of NG feeding. Elderly individual with much older age and who accepted NG feeding had higher possibilities of being hospitalized.In addition, older individual with NG feeding ones even hada longer hospital stay. The weak nutritional status of calf circumference and hemoglobin was another factor increasing their mortality. Based on the research findings, it’s recommended that it should be paid much attention to the nutritional status for elders living in thefacilities to reduce the complications, medical expenses, and death rate of being hospitalized, and to improve the quality of care.
