Efficacy of Functional Electrical Stimulation in Dysphagia of Nasopharyngeal Carcinoma Patients Post Radiation therapy: A Preliminary Study

Twenty post-radiation therapy (RT) Nasopharyngeal Carcinoma (NPC) patients suffering from dysphagia were randomly matched with dysphagia rating by Dysphagia Outcome and Severity Scale (DOSS) and divided into Functional Electrical Stimulation (FES) and home rehabilitation program groups. The subjects received subjective Quality of Life (QoL) questionnaire of swallowing and objective Videofluoroscopic Study (VFSS) evaluation before and after training. The parameters of subjective evaluation included QoL questionnaire of swallowing scores, and the parameters of objective evaluation included DOSS, Penetration-Aspiration Scale (Pen-Asp Scale), swallowing duration, hyoid moving measurement, and pharyngeal stasis. The FES group received 15 sessions of electrical stimulation on swallowing training, and home rehabilitation program group only performed swallowing exercise at home. The result showed general improvement in all of the swallowing parameters evaluated in FES group. Furthermore, there were significant improved in QoL score (p=0.003), hyoid moving duration of thin barium (p=0.001), hyoid moving speed of paste barium (p=0.028), and pyriform sinus stasis area of paste barium (p=0.026). The hyoid moving speed was significantly quicker (0.018) of thin barium in FES group than that in home rehabilitation program group after training. We concluded that FES had positive effects in dysphagia of NPC patients post RT. Further research is mandatory in defining its role in this group of patients.
