The Study of Voluntary Demotion – an Example of Nursing Staffs in a Government Hospital

Human resource development is the most important property of the developing organizations and societies. Nurses want to get nursing manager positions in excellent government hospitals. Promotion seems to be the ultimate goal everybody continually pursues. However, an abnormal human resource development situation was found in a certain government hospital. Nursing managers applied for demotion and the rate was increased. The purpose of this study was to explore the reasons why the nursing managers applied for demotion in order to find a way to keep the stability of human resource development and improve nursing quality and organizational effectiveness. Case studies method was used in this study. The data collection method included document review, interview, and observation. Eight nursing manages (included head nurses and assistant head nurses) who voluntarily applied for demotion during 2005 to 2007 were interviewed by seven open questions. The results showed that the participants had high-standard requirement to complete the missions during the time the hospital leaders proceeded the plans, such as: culture change, change with total quality management, and change with balanced scorecards, in order to found organizational culture. Following the higher value of the nursing supervisors, the work loading was increasing and resulted in that the participants received more stresses; such as, heavy work loading, role conflict, different leadership style, poor role function in family, and poor health. Therefore, the participants voluntarily applied for demotion to release from these stresses. The participants reported that they could have high-standard requirement to complete the missions, increase work efficiency, improve role function in family, and have time to take care their health after demotion. These are the reasons why the participants voluntarily applied for demotion. The findings suggest that the work loading needs to be adapted according to the organization and position requirements.
