The Effectiveness of Self-Management Program on Self-Care for Adult Asthmatics-- Example a Regional Teaching Hospital in Pingtung

Objective:Recognizing the importance of self-management in asthmatics care, this study attempts to assess the efficacy of a self-management plan on care improvement of asthmatics, and examine whether any difference of efficacy exists in those cases of asthmatics under self-management treatment program in a regional hospital in Pintung, Taiwan. Methods:This study was conducted by the Department of Thorax with its confiirmed asthmatic patients of a regional teaching hospital in Pintung. Purposive sampling is applied to categorize the gathered cases into general care and self-management groups. Patients completed a questionnaire on the first day of acceptance and again after six months of care. The questionnaire consists of six sub-scales of knowledge of asthma, behavior towards medical advice, social support, quality of life, self-report of asthma care, and clinical care records. Data gathered then were used to assess the outcome of the asthmatic patients who were involved in the self-management plan. Results:The study has collected 140 samples in total, with 70 cases in each group respectively. Six months after the intervention, the study results reveal several important information. (1). Level of knowledge of asthma has been significantly improved in the self-management group, of which higher than that in the general-care group. (2). Both the general-care and the self-management groups shows significant improvement in terms of medical advice complying, though the self-management group perform much higher. (3). None of these two groups has significant improvement in social support. (4). The self-management group has better performance in terms of quality of life in asthmatic patients, although the controlled group has significant improvement as well. (5). The self-management group obtained much better performance in varied aspects of asthma self-report. The self-management group significantly lowers the frequency of asthmatic attacks during the day and at night, better improvement in the asthma self-control, and more regular physician visiting than that of the general care group. (6) As the clinical records indicate, the self-management group has been much better off in the severity of asthma than that of the general-care group. (7). Ages, education levels, and years of asthma do not play significant impacts in any aspects of the improvement. (8). Both groups who were under either day or night asthmatic attacks obtain significant improvement in thier quality of life. (9). The self-management group with better asthma control confesses a better improvement in quality of life. (10). The self-management group presents better performance in terms of asthma knowledge, medical advice complying, and quality of life under asthmatic condition. Discussion and Suggestions:As the study results revealed, asthma patients under hospital care obtained significant improvement in their quality of life when complying medical advice. Asthma patients who participated in the self-management plan achieve even higher improvement in terms of knowledge of asthma, compliance with medical advice, and quality of life. The latter group also experiences fewer asthmatic attacks as well as improved clinical care records. This study provides evidence that self-management plan indeed help improving the efficacy of asthmatic patient management, effectively controlling the problematic disease by fully utilizing their potential, and accordingly improve their quality of life. The author wish the study result may serve as a reliable reference to healthcare professional in promoting the asthma self-management plans.
