Evaluation of Asthma Disease Management Program in a Medical Center

Popis: | 93 Research purpose: To understand self care ability, control of asthma and life quality of patient after they join the disease management self care program, in the medical center. This can provide as a guide to other medical organization. Research methodology: This study use generation study, evaluation questionnare for self care ability of asthma patient ( Include understanding about asthma, attitude, self ability and self management behavior), Juniper et al designed Asthma Quality of Life Questionnare - AQLQ), asthma controlling table and basic data of cases and relative items, a total of 4 portions. Patient from chest department of a medical center that join the “National Health Insurance” (National Health Insurance Disease Management Self-Care Program) were included in this study. They were analysis for different in self care ability, control of asthma attack and life quality. Research results: A total of 30 patients were included, after joining the disease management model, two tests were carried out to compare their different, each done at a 3 months interval. There were different in self-care ability, control of asthma attack and life quality after the intervention of disease management after the 3 months interval. But the second item in general self-care ability: “self management behavior” and detail questions about self-care ability showed that there were different after the intervention. After the hospital management model, those join asthma disease self care ability improved with time of intervention, but worse in second test about self-management behavior. Knowledge about asthma increase shortly after management, different in number of correct answer to various questions in measuring table, most improved in their second test. Their attitude did not change short term along with the management time. There was no significant different both in general or measuring table. Self efficiency improve in short term after management, mostly in first and second measuring table, most second test were getting worse. In life quality also no different after short term control in either general or separated by severity of disease group. From relativity, asthma control and life quality total mark and it second item were negatively correlation. Life quality and its second item were of positive correlation. Life quality and its second item environment stimulation and knowledge were negatively correlation. Possibly those with higher mark in knowledge were more able to sense the environmental stimulation, therefore the injury severity of life quality’s environmental stimulation were also more prominent. Discussion and Suggestions: This study showed that asthma disease management, should belong to long-term care, and teaching patient self-care is important issue in asthma disease control. From the standpoint of disease case manager, an effective disease management model should be a holistic design with different asthma teaching program to suit the various need of patient in order to improve the patient self care ability, decrease asthma attack symptoms and thus improve patient life quality. |
