Psychometric Evaluation of the Revised Fagerstrom Test for Nicotine Dependence

The Bureau of Health Promotion under the Department of Health in Taiwan concurrently employs a Chinese version of “ Fagerstrom Tolerance Questionnaire” (FTQ), developed by Karl-Olov Fagerstrom in 1978, as the evaluation tool for its trial program on outpatient smoking cessation treatment. However, this Chinese version has not yet been evaluated for the reliability and validity in Taiwan. Using quantitative and cross sectional research design, this paper aims to provide psychometric assessments on both Chinese FTQ and a revised FTND (Fagerstrom Test for Nicotine Dependence) which was developed by our research group based on literature studies and clinical experiences. 120 adult smokers and 120 non-smokers from different communities were randomly recruited and interviewed. During the interview, the participants were asked to fill out a Chinese FTQ and our revised FTND, and received a physiologically test on two biomarkers, i..e. their levels of expired carbon monoxide and saliva cotinine.. The data collected were then analyzed with SPSS 13.0 system. The validity of both Chinese FTQ and the revised FTND was evaluated through the examination of translation validity, the validity related criteria and known-groups validity criteria, while the value of Cronbach’s alpha was used to calculate the reliability of both questionnaires. Our research findings are as follows: (1) The translation validity: The content validity score/value of each individual question in the Chinese FTQ was 20 ~ 100%. The general content validity score of the Chinese FTQ as a whole was 75%, falling short of the standard score (80%). These figures revealed that the Chinese FTQ lacks valid translation.
