Alcohol Use Disorders among Non-psychiatric Inpatients in a General Hospital

Popis: | 91 Alcohol use disorders (AUDs) are common co-morbidity among inpatients in general hospitals, and cause excess mortality. The aims of this study are to investigate the prevalence and demographic correlates of AUDs in one general hospital and to evaluate the identification of patients with AUDs by medical staff. Five hundred and thirty eight non-psychiatric inpatients aged from 18 to 65 years old were randomly selected from one general hospital. In a two-phase design, Alcohol Use Disorders Identification Test (AUDIT) was used as a screening tool in the first phase and Schedules for Clinical Assessment in Neuropsychiatry (SCAN) as a diagnostic tool in the second phase. A total of 422 (78.4%) patients completed the first phase screen. Among them, 193 patients were AUDIT=0, 103 non-zero screen negative (AUDIT=1-5), and 126 screen positive (AUDIT6). Part of those screened negative and all of those screened positive were interviewed in the second phase. One hundred and twenty seven patients were interviewed by a psychiatrist using SCAN. Among them, 14 patients were found to have alcohol abuse and 45 patients to have alcohol dependence. Weighted one-year prevalence of alcohol abuse and alcohol dependence was 3.9% and 12.6%, respectively. Smoking, young, low education level and admission in internal medicine were significantly correlates with AUDs. The overall identification rate of AUDs by medical staff was 25.4% (0% for alcohol abuse and 30% for alcohol dependence). Patients with AUDs in internal medicine departments were more likely to be detected than those in non-internal medicine departments. Patients with lower AUDIT scores were easily neglected. Sensitivity and specificity of AUDIT at cut-off 6 to screen AUDs were 100% and 84%. In conclusion, around one sixth of non-psychiatric inpatients in general hospitals may have AUDs and are easily neglected by medical staff. AUDIT is a valid screen tool for AUDs in routine practice. |
