The process of forming empathy in nursing: a phenomenological perspective

Empathy is the natural process arising either unconsciously or consciously during interpersonal interaction. The purpose of this study is to understand how nurses unfold the experience of empathy during the process of nursing care. The investigator acted as a facilitator to participate in and observe the group psychotherapy in the acute psychiatric wards of two teaching hospitals. Each group psychotherapy was held once a week, twelve times in all, and the process was tape-recorded and transcribed into written documents subsequently. The investigator analyzed the transcripts of the group psychotherapy based on the perspective of phenomenology. The findings show that there are four forms of entering the process of empathy from the position of being a nurse: 1. Returning to face here and now from different time frames: (1) Projecting to the future, or (2) according to the past. 2. Entering the connecting relationship from the personal self. 3. Distinguishing the differences between self and others from the chaos: (1) The patient establishes a sense of reality with me. (2) I seem to be more like a patient than the patient is. (3) We hold different positions. (4) Why do you have this feeling? (5) I do not understand what they are talking about. 4. Approaching the patients'' world based on personal experiences, including (1) through the senses of the body, (2) via imagination, and (3)from the recall of previous experiences. In nursing practice, the nurse can purify the body from the space and distance created in the group. Meanwhile, through the dialogue and communication between the inner and outer world, the nurse can recognize the content of the covered-up self and the differences between self and others further. The experiences of body can become the medium of entering the inner worlds of others, and thus empathy can arise from turning to where and when the patients stand through self-adjustment. The meaning of nursing care can be derived from the humanitarian concern that empathy illuminates.
