An Empirical Study of Determinants of the Growth of Medical Care Expenditure─Application of Time Series and Threshold Model Approaches

This research uses national medical care data of Taiwan from 1954 to 1999 to construct macroeconomic time series and threshold models and to confer the determinants of the growth of medical care expenditure. Results indicate that, the demand-side includes per capita GDP and the ratio of insured, and the supply- side includes doctors and beds per capita and unemployment rate, are all factors that affect the growth of medical care expenditure. Based upon unit root and co-integration tests, all these factors appear to be stationary series of degree one and also have co-integrated relationship. This research also uses per capita GDP, the ratio of insured, doctors and beds per capita, and unemployment rate to be the threshold variables separately to test the structure changes of the medical expenditure. Results reveal that per capita GDP is the most important factor determining medical care expenditure, but the existence of medical insurance may mitigate this effect. Supply-induced-demand in the medical care market of Taiwan is not quite clear probably due to the macro-level data used in this study. Finally, the growth of unemployment rate increases medical care expenditure. During the high unemployment rates in recent periods, the government shall have taken this point of aspect into consideration.
