Popis: | 89 The major purposes of this research are : 1) to understand the differences in demographic characteristics, and medical conditions among the patients suffered from subarachnoid hemorrhage, intracerebral hemorrhage, and cerebral infarction ; 2) to compare the hospitalization charges and its predicting factors among the three group subjects ; 3) to compare the first-year direct costs, indirect costs, and total costs among the three group subjects. The research is a cross-sectional, retrospective study. The study subjects are based from the first-ever stroke hospitalization patients in a medical center. The study sample were 11 in subarachnoid hemorrhage group, 31 in intracerebral hemorrhage group, and 106 in cerebral infarction group. The survey was performed by telephone questionnarie. Data included two parts. Part A included medical costs and medical information which were obtained from insurance claim files and medical records. Part B included “ out of pocket “ expenses within one year after stroke onset, and the current modified Barthel ADL index, which were derived from survey data. The results conclude that the mean age was youngest in subarachnoid hemorrhage group, with intracerebral hemorrhage, and cerebral infarction in increasing order. The average hospital stay costs were NTD$333,625 ; $154,441 ; $91,242 in subarachnoid hemorrhage, intracerebral hemorrhage, and cerebral infarction respectively. Room charges, drugs, and evaluations were the highest in the cost structures for hospital stay. The major predicting factors of hospital costs are type of stroke, length of hospital stay, brain surgery, tracheostomy, and moderate to severe dependence in Functional outcome. The one year direct costs are 431,579 ; 402,043 ; 286,212, in subarachnoid hemorrhage, intracerebral hemorrhage, and cerebral infarction respectively. The one year total costs, in the same order, are 602,513 ; 718,191 ; and 524,382. Since hospital costs were affected by length of stay, we suggested to control the hospital stay without compromising quality of care in those patients without good family support. Stroke is a major disease, which consumes certain percentage of social resource. The government should emphasize on disease prevention and risk modification in order to decrease the incidence of stroke . |
