Analysis of in patient care resource utilization by low birth weight premature infants

Since a largely medical resource usage is accounted for by the initial hospitalization after birth; therefore this study was under taken to determine the resource that is used by initial hospital care for infants with low birth weight and to explore the factors that contribute to the resource utilization. A retrospective study was designed to collect data from the Tertiary Hospital in northern Taiwan. From 1998 to 2000, all infants in the study were under 37 weeks gestation, weighed less than 2,500g at birth. Variables were carefully examined using t-test, ANOVA, and multiple linear regression for their independent effects on hospitalization resources. The results have shown that birth weight, number of complications, and severity of illness were related to initial hospital care resource utilization among surviving infants but not among non-survivors. The average hospitalization resource utilization per survival infant was twice as high for infants weighing under 750g (cost approximately NT$1,586,000、LOS is 146 days、NICU LOS is about 105 days) as for the next higher group of infants weighing between 750-999g. The cost was gradient dropped to approx. NT$94,000、14 days of LOS and 4 days of NICU LOS per survival infant with birth weight between 2000-2500g. The results provide guidance for health care administration and practice to have an appropriate policy for improving the welfare of premature infants with low birth weight and pregnant women.
