Popis: | 87 A Study of British Care Management Model--Three Cases of Elder Community Care Project Abstract Welfare communitization (community-oriented welfare service) is one of important tasks amid Taiwan Welfare service project for the time being. Among the said tasks, community care is not only the key work but also the new trend in the field of welfare service. Both the experience of community care policy in British and the result of research by domestic scholars indicated simultaneously that the care management is a prerequisite working method for carrying out the community care. This study, by analyzing three British community care projects for the elderly-Kent Community Care Project, Darlington Project and Scotland EPIC Project, aims to understand the factors influencing results of care management. Their developments, practices, characteristics and results are analyzed and compared. Some lessons and implications from this study are provided for developing a care management model in Taiwan. 一、Factors facilitating the implementation of care management to success and leading to failure are as follows: (一)Factors facilitating the implementation of care management to success 1. With definite policy and feasible goal. 2. Appropriate span and budget for the project. 3. With well management and definite content to be worded out. 4. Well system and team in the project and integrating the objectives and resources of each service systems. 5. Care managers with professional background in social work and good experience in practice. 6. Reasonable caseload for care managers. 7. Sufficient budgeting that allocating for care managers. 8. To employ sufficient community care workers. 9. To promote client’s confidence in project and the care managers. 10. To take multiple and complete assessment. (二)Factors which hinder the implementation of care management from success: 1. Indefinite job specification and unequal allocation of task. 2. Without common sense and poor communication between project participants with varied professional backgrounds in care management. 3. Too short span for implementing project. 4. Insufficient initiative and too conservative among care managers. 5. No consistent assessment as a result of different backgrounds of care manager. 6. Too bureaucratic and inefficient as a result lacking flexibility. 7. Insufficient efficiency because of inconsistent assessments made by various service providers. 8. Lack of client’s confidence on project. 9. Too slow on referrals as a result of poor operation on screening system. 二、Suggestions to the project of care management in Taiwan is as follow: (一) In regard to the implement of care management: 1. The carrying out of care management must be based on a good plan. 2. Taking financial and implementing plan into careful consideration. 3. To take advantage of social work profession in care management. 4. To link up original resources and new type of service. 5. To carry out the strategy of the purchaser/provider split. 6. To put emphasis on the development of the project. 7. To encourage care manager in building up a initiative environment. (二) In regard to the cooperation between service systems: 1. To integrate the objectives and resources of institutions and services. 2. Cooperation between multidisciplinary must base on common viewpoint with mutual trust and respect to each other. 3. To reinforce the development and distribution of equilibrium welfare resources. 4. To promote integration and cooperation between institutes and multi-disciplinary. (三) In regard to service receiver: 1. To cultivate ability of service users in participation and making choice. 2.
