Post-operative Comfort Following Dental Treatment under General Anesthesia

Popis: Purpose: This randomized, prospective, blinded study compared comfort in children following dental treatment under general anesthesia (GA) using 1 of 2 established analgesia methods. Methods: Patients, aged 4 to 7 years, were randomly assigned to a control group (intravenous (IV) analgesics) or experimental group (intravenous analgesics with local anesthetic (LA) gingival infiltrations). During recovery from surgery, Faces, Legs, Activity, Cry, and Consolability (FLACC) scores were recorded upon regaining consciousness and retaken every 15 minutes until discharge. Subject pain incidence (defined as no pain=FLACC 0, pain=FLACC >1), and moderate-severe pain incidence (defined as no-mild pain=FLACC4) was analyzed using logistic regression (N=75). A priori, it was determined that a difference in pain incidence > 10% was clinically significant. Results: Patients randomized to the experimental group had significantly lower pain incidence than the control group by 16% (LA=.16, Control=.32, P=.05). The experimental and control groups were not significantly different for moderate/severe pain incidence (LA=.08, Control=.21, P=.15) Presence/absence of stainless steel crowns, extractions, and/or pulpotomies did not significantly affect pain incidence.
