Endothelial function in patients with arterial hypertension and initial clinical symptoms of shronic serebral insufficiency

Popis: Prevention of chronic cerebral blood supply insufficiency is theleading aspect in prevention of cerebrovascular diseases. Initialstages of these changes are of special interest, with endothelialdysfunction playing the major role. The aim of the study is toassess the function of endothelium in patients with initialstages of chronic cerebral blood supply insufficiency and noncomplicatedhypertension. We included 48 men and 61 women(mean age 57.45.8 years) to perform complex standardizedassessment. Patients with history of stroke or acute coronarysyndrome, head trauma or other severe disease, or stenosis ofcervical arteries 30% were excluded. We performed clinicalevaluation with assessment of neurological deficit and duplexultrasound of cervical arteries, echocardiography, 24-hourblood pressure monitoring, brain MRI or CT and complexneuropsychological testing. Endothelial function was assessedwith functional tests (hemoreological and vasoactive cuff test,test of cerebrovascular reactivity with nitroglycerine). We testedthe level of Von Willebrand factor and homocystein as biomarkersof endothelial function. The rate of endothelial dysfunctionin patients with non-complicated hypertension was64%, and high agreement rate of both cuff tests was revealed.The hemoreological cuff test was difficult to perform, and itsresults were highly dependent on the initial values. Prospectivestudies are needed to estimate the prognostic value of endothelialdysfunction for the progression of the chronic cerebralblood supply insufficiency.
