A 36-year-old woman with cannabinoid hyperemesis syndrome presented with chest pain and was found to have single-vessel coronary artery disease and an aortic mural thrombus. This case describes unique management with coronary artery bypass and surgical thrombectomy because of the patient’s inability to tolerate uninterrupted antiplatelet therapy given her cannabinoid hyperemesis syndrome.
