THE BOSTON TYPE I K-PRO IMPLANTATION RESULTS IN PATIENTS WITH BURN AND DYSTROPHIC LEUKOMA OF 4-5TH STAGE

Popis: Purpose. To analyze the Boston type I K-pro implantation results in patients with burn and dystrophic leukoma of 4-5th stage. Materials and methods. We implanted the Boston K-pro in 6 patients (6 eyes), among them – 4 patients (4 eyes) with burn vascular leukoma of 4-5th stage, 2 patients (2 eyes) with epithelial-endothelial corneal dystrophy. All of them underwent functional, electrophysiological, ultrasonic, bacteriological and serological diagnostic procedures pre-op. and in different post-op. periods and in cases of complications. Results. In all cases we revealed VA improvement within the limits from 0.2 to 1.0. During the first month of follow-up no postop complications were detected. Within next 3 months the stromal necrosis was a main complication in 4 of 6 patients: one case was caused by infectious keratouveitis, in all other cases we observed aseptic necrosis. In one patient we removed the prosthesis. After 1-year follow-up there was a group of complications not related with necrosis of corneal tissue: secondary cataract (1), cataract (1), glaucoma decompensation with retroprosthetic membrane formation (1). During next year (up to 2nd year of follow-up) the main complication again was aseptic stromal necrosis (in 2 of 5 patients), in one case the k-pro was removed. Conclusion. The gained results of keratoprosthetics with the Boston type I k-pro show us high anatomic and functional outcomes that vary depending on initial eye condition. In patients with burn leukoma the k-pro retention was achieved in 50% of cases, in dystrophic leukoma – in 100% of cases. The method is an alternative to other clinically used keratoprosthesis models.
