A 32-year-old man presented with a fairly large intramedullary mass lesion involving D5 to L1 segment of spinal cord. It was hyperintense on both T1Weighted (W) and T2W sequences, with signal suppression on fat-saturation images leading to the diagnosis of dermoid cyst (with extensive intramedullary occupancy of dorsolumbar cord) with multiple tiny fat deposits along proximal cervicodorsal cord.
