Introduction: the gingival enlargement is the increase of the size of the gingiva caused by diverse factors, between which are the drugs, mainly the antihypertensive, immunosuppressive and anticonvulsant medications. Between the first ones, the nifedipine, an antagonist of the calcium, is one of the most frequently indicated. Clinical case: A 62-year-old-male patient with arterial hypertension treated with nifedipine for more of fifteenth years, who presented chronic gingival enlargement in the area of the lateral incisors and cuspids of the maxilla. External bevel gingivectomy with electric scalpel was carried out, with appropriate evolution and good healing. Conclusion: the gingival enlargement caused by nifedipine merits treatment that includes suspension of the drug and oral hygiene. In some cases, gingivectomy must be done.
