Abstract Introduction Internal medicine (IM) residents must learn key collaborative competencies early in their postgraduate IM training to be effective in the complex inpatient IM environment. To allow new IM residents to learn health care professional (HCP) roles, we traditionally held a didactic session where HCPs introduced themselves and their roles to the new residents. These sessions were poorly rated by the residents, being lengthy and often focusing on the don'ts versus dos, leading to an initial impression of a conflict-laden relationship versus a teamwork relationship. We therefore developed the Safe Discharge Challenge, which has been incorporated into the IM orientation in PGY1 since 2012. Methods This is an interactive approach to learning roles of other HCPs using fictitious cases. First-year IM residents are divided into teams of six and given a virtual paper patient to discharge. After a short 5-minute introduction to helpful teamwork tools, as well as time to read their cases and discuss the needs of the patient, the residents are directed to the offices of the HCP who could assist them. After the challenge, the residents present their discharge plans to the faculty and the HCP to ensure all needs are met and readmission avoided. Results In 2012 the session was rated as very valuable by the IM residents (mean 4.3 on a 5-point Likert scale). This was improved from the 2011 didactic session (mean 3.3). Since 2012, the Safe Discharge Challenge described here has been used as the orientation tool for new IM PGY1s; feedback has been very positive from both residents and HCPs. Discussion This resource successfully serves as an orientation to the role of HCPs during a PGY1 IM transition bootcamp. As a result of this knowledge, learners are better able to collaborate with other HCPs on an IM ward. |
