Abstract Introduction This anaphylaxis simulation scenario is intended for a small, interdisciplinary mock code session to be performed in a hospital patient care area. Methods The resource includes an instructor guide, supplemental materials, and a standardized patient training form. The instructor guide contains the scenario and a guide to debriefing that covers team communication and medical management. The supplemental materials are labs, X-rays, and mock chart material and images to help perform the case. Our standardized patient training form ensures that the standardized patients are aware of and can achieve the performance we want in order to highlight the communications goal of our team scenario. Results We have been performing this scenario as part of our in-situ mock code program for the past several years. As a result, residents and nurses express improved ability to recognize shock, allergic reaction, and anaphylaxis and initiate appropriate initial treatment. Repeat performance by both physicians and nurses has resulted in faster recognition of anaphylaxis and faster administration of epinephrine in the scenario. Both resident physicians and nurses report improved understanding of hospital resources. They have also shown better understanding of the physical resources available to them on med-surge units. Finally, implementing this scenario has resulted in improved communication between physician and nursing staff during situations of medical emergency and escalation of care. Discussion We utilize pediatric residents, pediatric floor nurses, and inpatient pharmacists for this scenario, but it could be easily adapted for use with other learners or in different settings. The tool has helped standardize team debriefing of the scenario as we use all of our ICU faculty as instructors. We have seen our faculty improve their ability to effectively involve all team members in the debrief session, lead discussion on the communication that occurred during the mock code, and encourage a safe environment for the team to discuss their performance. The information on medical management also helps to ensure that our instructors provide more standardized, focused teaching points to the team in the limited time we have. |
