Popis: Dislocations of the proximal interphalangeal joint of the lesser toe are considered a rare injury in children and uncommon in adults, it is important to consider the possibility of a buttonhole phenomenon if unsuccessful close reduction happens. Open reduction is the appropriate management in such cases. In this statement, we present unusual isolated acute irreducible 5th toe proximal interphalangeal joint dislocation of a nine years old boy who underwent open reduction and Kirschner wires (K-wires) fixation of the 5th PIP joint due to inter positioning of the distal part of the proximal interphalangeal joint through the medial capsule preventing the reduction.
