Popis: Introduction: the dentigerous cyst is a pathological entity which occurs in the mandible or maxilla, always related to the crown of an included tooth, with a higher incidence in males between 20-30 years of age. Case presentation: the clinical case of a 73-year-old female patient is presented, who comes to the Maxillofacial Surgery consultation, out of concern at presenting a moderate increase in volume in the jaw, with a sensation of cramp and pain that gradually increased her intensity; with evolution of approximately one year. Radiographic examination reveals a well-defined radiolucent image. Included in this image is a retained tooth that corresponds to the lower right third molar. Surgical excision was planned, resulting in a definitive histopathological diagnosis: dentigerous cyst. The patient evolved satisfactorily during the postoperative period both clinically and radiographically. Conclusions: an early diagnosis of this condition can prevent bone expansion, facial asymmetry and severe pain, as well as transforming into malignant neoplastic tumors. Subsequent corroboration of the presumptive diagnosis through the histopathological study is essential and short and long-term follow-ups of the patient are recommended.
