General Anesthesia for a Laparoscopic Nissen Fundoplication in an Infant After Norwood Repair for Hypoplastic Left Heart Syndrome

Popis: Abstract This problem-based learning discussion (PBLD) is designed to facilitate a group discussion among learners using a real clinical case scenario to consolidate self-directed learning. The clinical case scenario involves a neonate born with hypoplastic left heart syndrome (HLHS), which is status post-Norwood repair. Due to feeding difficulties, the patient is scheduled for a laparoscopic Nissen fundoplication and gastrostomy tube placement. This patient and surgery demonstrate several anesthetic challenges. After presentation of the case stem, the instructor moderates a student-centered group discussion, using a series of questions and a review of relevant literature. The author encountered this complex clinical case scenario involving several anesthetic challenges multiple times in an academic pediatric anesthesia setting. It was developed as a PBLD and was presented at the Society for Pediatric Anesthesia winter meeting in San Diego, CA, in March 2010. A variety of anesthesia care providers, including anesthesiology trainees and anesthesiologists from academic institutions and private practice, participated in the discussion, and the evaluations were uniformly very positive. The case has also been used at the author's current institution and two previous institutions with anesthesiology residents and pediatric anesthesiology fellows as a part of their PBLD curriculum and clinical teaching with very positive evaluations. After completion of this module, the learner would not be competent to provide anesthesia for one of these surgeries. The learner would have a better understanding of the physiology and anesthetic challenges involved in the care of these patients.
