Introduction: Excisional toenail surgery is a common treatment employed by Podiatric Surgeons for the management of onychocryptosis, with risks generally considered to be low. We report a case of an organized post-operative hematoma causing wound complications, subsequent revisional surgery and reflect on national audit data. Case presentation: An elderly female was referred to the podiatric surgery department due to recurrent onychocryptoses of both great toenail nails having previously undergone two failed partial nail avulsions with phenol chemical matrixectomy. A Winograd procedure was performed under local anesthesia, excising the affected nail borders and corresponding matrix. One healed uneventfully however the other developed post-operative infection, dehiscence and fluctant mass at the proximal incision. Investigations were indeterminate and so revisional surgery was required identifying an organized hematoma. Excision of the lesion with primary closure healed as expected. Initial post-operative cosmesis of the nail plate and overall short term outcome was poor compared to the contralateral side reflecting patient satisfaction. By one year follow up, the nail had regrown with an aesthetically pleasing nail plate, remained asymptomatic with high patient satisfaction. Conclusion: Hematoma formation is commonly listed as a complication in an array of surgical procedures, however not specifically associated with excisional toenail surgery. To the authors knowledge, this is the first reported case in this instance and the intent of this report to demonstrate this atypical finding to surgical colleagues. |
