Abstract Over the past 15 years, there has been a shift in medical trainee education assessment and evaluation. The landmark change included the now well-known six core competencies proposed by the ACGME in 2001: patient care, medical knowledge, practice-based learning and improvement, interpersonal and communication skills, professionalism, and systems-based practice. This has evolved into a formalized milestone reporting system required by the ACGME and Next Accreditation System in an effort to develop more thoughtful and standardized evaluations of trainees. While the Reporting Milestones system is an improvement on previous evaluation systems, it is not always easily applicable across the varying specialties within graduate medical education. Moreover, the evaluations do not include specific competencies or benchmarks within the field of gastroenterology (GI). To provide guidance to GI fellowship program directors to assist meeting the Reporting Milestone guidelines, a collaborative committee called the Oversight Working Network (OWN) was established in 2013. The OWN group developed a toolbox of Entrustable Professional Activities (EPAs) specifically for GI fellowship training programs in order to provide a template of GI-specific fellow assessments. The OWN toolbox consists of 13 EPAs that can be applied to an individual GI training program in order to meet the subspecialty Reporting Milestones requirement. This module outlines how the University of Chicago Medical Center GI training program utilized the 13 EPAs published by the OWN group to create eight specific fellow assessment tools as they relate to existing service rotations. Evaluators receive an email notification to complete the evaluation immediately at the end of the trainees' rotation. This email includes instructions on how to access the evaluations through the electronic medical education management system used at our institution. At the time of implementation, all faculty evaluators (20 core faculty) were informed about the new evaluations. New clinical faculty joining the institution will be instructed on the use of evaluations as part of the standard orientation process. From December 2014 through May 2015, 12 trainees have been assessed using these evaluations. While we do not have formal feedback yet, initial responses from fellows and faculty have been positive due to the GI-specific nature of the evaluations and more useful feedback on how to improve. |
