THE RESULTS OF APPLYING THE METHODOLOGY EXTENDED ENDARTERECTOMY OF THE FEMOROPOPLITEAL ARTERY SEGMENT

Popis: Purpose: To improve the results of surgical treatment of patients with combined (multi-storied) occlusions, stenosis and thrombosis of the femoro-popliteal arterial segment by introducing a new method of reconstruction.Materials and Methods: 278 case histories were compared. Compared two types of reconstructive surgery: autovenous femoropopliteal bypass and extended endarterectomy.Results: In the earlier postoperative period the extended endarterectomy method allows you to get a positive result in 93,8% of observations, femoropopliteal and femoro-femoral bypass allows you to get a similar result in 81,2% of cases. In the late postoperative period endarterectomy can save 87,5% of the operated limbs, the thrombosis of superficial femoral artery was18,8%, and 78,8% in cases of autovenous bypass, the thrombosis of bypass in 57,6% of cases.Summary: The technique of extended endarterectomy allows to restore circulation in its entirety and this method is theoperation of choice. When performing an extended endarterectomy we save the bloodstream through the own arterial channel,preserving functioning collaterals. In the cases of thrombosis of the superficial femoral artery we rarely need to perform anamputation of the limb.
