RECONSTRUCTION OF DAMAGES TO THE ROTATOR AND BICIPITAL ELEMENTS OF THE ROTATOR-BICIPITAL COMPLEX IN THE TREATMENT OF THE PROXIMAL SHOULDER

Popis: | Background. The topicality of the topic is determined by the frequency of injuries of the proximal shoulder, which account for up to 80% of all bone injuries of the upper limb girdle, while approaches to their reconstruction are unresolved. When analyzing adverse treatment outcomes retrospectively, insufficiently accurate reposition of bone fragments, underestimation of soft tissue lesions, and osteosynthesis without prophylaxis of impingement are often noted, but the measures for their prevention are not straightforward. Objective: to improve the results of surgical treatment of patients with proximal shoulder fractures. Material and methods. The material of the study is the evaluation of the results of treatment of 512 patients aged from 18 to 87 years old, operated from 2004 to 2018 using the tactical approaches and methods developed by the authors to restore the rotator and bicipital elements of the rotator-bicipital complex. Research results. The use of the developed tactical approaches and methods enabled to achieve a favorable outcome of treatment in 503 (98.2%) patients. Conclusions. Reconstruction of injuries of the rotator-bicipital complex is a promising way to improve the results of treatment in patients with fractures of the proximal shoulder. |
