Laboratory? or Fertility? in Ectopic Pregnancy Management

Popis: INTRODUCTION: The aim of this study is to evaluate diagnostic and treatment modalities with our treatment success rates of patients treated due to ectopic pregnancy METHODS: 111 patients of 140 patients treated due to ectopic pregnancy whose information could be accessed were evaluated considering; age, gravidity, contraceptive method, history of pelvic surgery, hemodinamically stability, first β-hCG values, presence of free abdominal fluid, ultrasonographic findings suggesting adnexal masses, the number of treatment approaches as wait and see, Methotrexate and surgery in time period between June2013-December2016 RESULTS: Mean age of the patients was 29-5,6, previous pregnancies with abortion 12,6%, cesarean 39,6%, normal vaginal delivery 3,6%, previous surgery due to ectopic pregnancy 17,1%. 21,6% of the patients were with hypovolemic findings and 78,4% of the patients were hemodynamically stable without rupture findings in the first attendance. In initial management, 23,4% of the patients had wait and see treatment, 55% (n=61) of the patients had medical treatment, 21,6% (n=24) of them had surgery. Success rate was 77% in the patients treated with wait and see and 64% was in the patients treated with single dose Methotrexate. Surgery was performed to two patients with failed two dose of Methotrexate treatment. 75% salpingectomy19,2%, salpingostomy 2%, salpingotomy and 3,8%, ovarian wedge resection was performed to 52 patients with surgical treatment. DISCUSSION AND CONCLUSION: Methotrexate and wait and see treatment modalities can be offered as the first choise to the patients without rupture findings, and stable conditions. In surgical treatment, fertility preserving surgeries should be the first choise considering vital findings and fertility desire.
