The hemostasis-regulating function and metabolism of liver in diabetes mellitus (literature review)

Popis: Studying the role of the liver in coagulopathy in diabetes mellitus is of great interest from the point of view of the complications frequency associated with this condition. The article provides a review of modern literary sources covering the mechanisms of changes in liver function in diabetes mellitus in terms of the inclusion of the plasma component of the hemostasis system. The factors influencing the quantity and quality of production of plasma coagulation factors under conditions of hyperglycemia and insulin resistance are considered. In the course of hypercoagulemia, patient innovation was demonstrated, such as diabetes mellitus, the level of hyperglycemia and its controllability. The following are considered as the main indicators of increasing the production of plasma hemostasis factors: the synthesis of pro-inflammatory cytokines by adipocytes, endothelium and cells of the immune system; hormonal drugs on the liver and features of hepatic insulin resistance; endothelial dysfunction. A decrease in the quantity and quality of plasma hemostasis factors associated with the progression of NAFLD to liver fibrosis and cirrhosis; predisposition of patients with diabetes mellitus to the development of diseases of the biliary system and changes in the composition of intestinal microbes, which leads to disruption of vitamin K metabolism.
