| Popis: | Following the establishment of a discharge working group, several concerns were raised about the management of multidisciplinary team (MDT) discharge meetings throughout the trust. A project was established to observe MDT meetings, identify good practice and produce practice guidance to improve standards and achieve consistency throughout the organisation. |
