Transsternal bilateral thoracotomy ("clam shell" incision) is an approach that allows wide access to both sides of the thorax. Its use dates from the earliest period of cardiac surgery, and it fell into disuse in the face of the advantages of a mid-level sternal approach. The "clam shell" incision was revived, however, with the introduction of lung transplantation. We describe 2 cases of bilateral pneumothorax treated by this surgical route. |
