[Appropriate administration of anticoagulants and antiplatelets]

Popis: For prevention of stroke in patients with nonvalvular atrial fibrillation, novel oral anticoagulants are superior or non-inferior to warfarin in efficacy, safety and easiness to use. Incidences of hemorrhagic stroke or intracranial hemorrhage are much lower in novel oral anticoagulants compared to those of warfarin. CHADS2 score or CHA2DS2-VASc score are used for evaluation of stroke risk and HAS-BLED score are for major bleeding. Antiplatelet agents are administered to avoid non-cardioembolic stroke. Clopidogrel and cilostazol are superior to aspirin in prevention of atherothrombosis and stroke, respectively. In treatment with aspirin, we should pay attention to gastrointestinal bleeding and intracranial hemorrhage, which are more frequently seen in Japanese than in people in western countries.
