The value of ultrasound examination of the ankle joints and tendons in systemic lupus erythematosus

Popis: Cilj Cilj ove studije je bilo odrediti učestalost i težinu ultrazvučnih promjena zglobova i tetiva gležnja u SLE te korelirati rezultate s kliničkim pregledom, mjerama aktivnosti bolesti i laboratorijskim parametrima. Ispitanici I metode ----- U studiju je bilo uključeno 60 uzastopnih pacijenata sa SLE kojima je isti dan učinjen klinički pregled, laboratorijska obrada te pregled zglobova ultrazvukom visoke rezolucije i power Dopplerom. Obostrano su pregledani: tibiotalarni (TT) i subtalarni (ST) zglobovi, drugi i treći metakarpofalangealni, drugi i treći proksimalni interfalangealni, drugi i treći metatarzofalangealni zglobovi, radiokarpalni zglobovi te tetive gležnja.----- Rezultati Ultrazvučne promjene zglobova gležnja su bile prisutne u 53,3% bolesnika. TT zglobovi su bili zahvaćeni u 43,3%, a ST zglobovi u 26,7% bolesnika. Zglobni izljev u TT zglobu je bio najčešći nalaz, prisutan u 22,9% pregledanih zglobova, zatim sinovijalna hipertrofija, prisutna u 7,5% zglobova. Polovica asimptomatskih bolesnika je imala prisutnu ultrazvučnu promjenu zgloba gležnja. Ultrazvučne promjene na tetivama bile su prisutne u 21,6% asimptomtskih bolesnika. Nije bilo značajne povezanosti između ultrazvučnih nalaza i upalnih parametara aktivnosti bolesti, ali našli smo slabu pozitivnu korelaciju između upalnog zbroja gležnja i ECLAM-a (r=0.27830, p=0.0313) te SLEDAI 2K (r=0.21557, p=0.098). ----- Zaključak Studija je pokazala visoku pojavnost ultrazvučnih upalnih promjena zgloba gležnja u simptomatskih i asimptomatskih pacijenata sa SLE.
Objective To assess the prevalence and severity of ankle joint and tendon changes in systemic lupus erythematosus (SLE) patients by using ultrasound and to correlate the findings with the physical examination, disease activity scores, and laboratory parameters. ----- Patients and Methods Sixty consecutive SLE patients were enrolled in the study. All patients underwent clinical evaluation, laboratory tests, and high-resolution ultrasound with power Doppler on the same day. Bilateral tibiotalar (TT) joint, subtalar (ST) joints, hand, wrist and foot joints, and ankle tendons were assessed. ----- Results US ankle joint involvement was present in 53.3% of patients. TT joints were affected in 43.3% and ST joints in 26.7% of patients. TT joint effusion was the most frequent finding, present in 22.9% of examined joints. As many as 50% of asymptomatic patients had pathological ultrasound findings in ankle joints. Ultrasound changes on tendons were present in 21,6% of asymptomatic patients. There were no significant correlations between ultrasound findings and inflammatory parameters of disease activity, but we found a weak positive correlation between the TAUSS and ECLAM (r=0.27830, p=0.0313) and SLEDAI 2K (r=0.21557, p=0.098). ----- Conclusion This study showed a high prevalence of ultrasound verified inflammatory ankle joint changes in symptomatic and asymptomatic SLE patients.
