Popis: UVOD: Koštani morfogenetski protein–7 (od engl. Bone Morphogenetic Protein-7) ima ključnu ulogu tijekom embrionalnog razdoblja razvoja bubrega. Osnovna mu je zadaća zaštita bubrega od akutne ozljede i razvoja kronične bubrežne bolesti, sprečavanjem razvoja upalnih procesa i fibroznih promjena. Miševi koji ga nemaju u genomu umiru prvi dan po okotu u slici uremije. Dijabetička nefropatija je vodeći uzrok kronične bubrežne bolesti i jedan je od najčešćih razloga za nadomještavanje bubrežne funkcije u Hrvatskoj, Europi i Sjedinjenim Američkim Državama. Prema dosadašnjim istraživanja, poglavito na animalnim modelima, obilježena je smanjenim izražajem BMP-7. CILJ ISTRAŽIVANJA: Cilj doktorskog rada bio je istražiti izraženost BMP-7 u različitim stadijima dijabetičke nefropatije, pokazati da li postoji povezanost s određenim demografskim čimbenicima, te u drugom dijelu rada, istražiti izraženost BMP-7 u bioptatima bubrega bolesnika s dijabetičkom nefropatijom. Istraživanjem na humanom modelu željeli smo potvrditi hipotezu da se izražaj BMP-7 mijenja napredovanjem bolesti. ISPITANICI I METODE: Istraživanje se sastojalo od dva dijela. U prvom dijelu, ELISA testom na uzorku od 120 ispitanika koji su bili podijeljeni u 4 skupine ovisno o procijenjenoj glomerularnoj filtraciji, izmjerena je koncentracija BMP-7 u serumu. Prvu skupinu činili su ispitanici sa šećernom bolešću tip 2 s početnim bubrežnim oštećenjem, drugu bolesnici u predzavršnom stadiju, treću skupinu bolesnici na hemodijalizi, dok je četvrta skupina bila kontrolna skupina zdravih dobrovoljaca. U drugom dijelu, istražen je izražaj BMP-7 u 30 uzoraka bioptata bubrega bolesnika s potvrđenom dijabetičkom nefropatijom, a kontrolnu skupinu činili su uzorci zdravog tkiva 12 bubrega koji su dobiveni tijekom nefrektomije bolesnika s lokaliziranim tumorom bubrega. Uzorci su izuzeti s najudaljenijega mjesta od tumora. REZULTATI: Ovim istraživanjem pokazano je da se izražaj BMP-7 mijenja napredovanjem bubrežne bolesti. Statistički značajno veće vrijednosti BMP-7 izmjerene su u skupini bolesnika sa završnim stadijem kronične bubrežne bolesti koji se liječe hemodijalizom u odnosu na druge dvije ispitivane skupine, kao i kontrolnu skupinu. INTRODUCTION: Bone morphogenetic protein-7 (BMP-7) plays a crucial role during the embryonic period of kidney development. Its basic role is to protect the kidney from acute and chronic failure, preventing the development of inflammatory and fibrotic changes. Mice at which it was expelled from the genome, die first day after litter of uremia. Diabetic nephropathy is the leading cause of chronic kidney disease and one of the most common reasons for renal replacement therapy in Croatia, Europe and the United States. According to previous research, especially in animal models, it is characterized by reduced expression of BMP-7. AIM: The aim of this Thesis was to investigate the expression of BMP-7 in different stages of diabetic nephropathy, demonstrate whether there is a correlation with certain demographic factors, and, in the second part of the work, to investigate the expression of BMP-7 in kidney biopsy in patients with diabetic nephropathy. In study on human model, we wanted to confirm the hypothesis that the expression of BMP-7 is changed with disease progression. METHODS: The research consisted of two sections. In the first section, by ELISA test on a sample of 120 patients, who were divided into four groups depending on the estimated glomerular filtration rate, we measured the concentration of BMP-7 in serum. In the first group there were subjects with diabetes mellitus type 2 with the initial renal impairment, at the second group the preterminal patients were, a third group was made of patients on hemodialysis, while the fourth group was the control group of healthy volunteers. In the second section, we studied the expression of BMP-7 in the kidney biopsy samples from 30 patients with confirmed diabetic nephropathy, and the control group was consisted of healthy tissue samples 12 the kidney obtained during nephrectomy patients with localized the kidney tumors. Samples were exempted from the most remote place of the tumor. RESULTS: This research showed that the expression of BMP-7 is changed with the progression of kidney disease. Statistically higher levels of BMP-7 were measured in a group of patients with end stage chronic kidney disease who are treated with hemodialysis, as compared to the other two test groups and control group. In the same group of patients we find statistically significantly different values of BMP-7 in correlation with fundus examination.
