Efficacy and safety of oral and inhalational commercial beta-glucan products: systematic review of randomized controlled trials : diploma thesis

Popis: Premda beta-glukane možemo pronaći u komercijalnim preparatima, ali i u različitim namirnicama obogaćenim beta-glukanima, analiza preparata u kojima je vrsta i doza beta-glukana nejasna ne može pružiti jasne smjernice za praksu. Stoga je cilj ovog istraživanja sinteza svih rezultata učinkovitosti i sigurnosti komercijalnih oralnih i inhalacijskih proizvoda beta-glukana. Proveden je sustavni pregled randomiziranih kontroliranih studija o djelovanju komercijalnih proizvoda beta-glukana na zdravlje čovjeka. Pretražene su baze MEDLINE i CENTRAL. Dva autora neovisno su pregledala naslove i sažetke dobivene pretraživanjem, cjelovite tekstove relevantnih istraživanja i izvadila podatke iz uključenih istraživanja. Korišten je Cochraneov alat za procjenu rizika od pristranosti. Uključeno je 28 randomiziranih kontroliranih studija koje su ispitivale djelovanje oralnih ili inhalacijskih komercijalnih pripravaka beta-glukana na zdravlje čovjeka. Dio istraživanja proveden je na odraslima, a dio na djeci. Analiziran je utjecaj beta-glukana na različite aspekte zdravlja bolesnih i zdravih osoba. Studije su prosječno imale samo 33 ispitanika po skupini, imale su visok ili nejasan rizik od pristranosti na barem jednoj domeni, niti jedna nije navela zaključak o sigurnosti u sažetku, i koristile su veliki broj mjera ishoda od kojih su mnogi surogatne mjere. Većina uključenih istraživanja zaključuje da beta-glukan ima povoljno djelovanje na analizirane mjere ishoda. Dokazi iz randomiziranih kontroliranih studija o djelovanju komercijalnih oralnih ili inhalacijskih pripravaka beta glukana vrlo su loše kvalitete, uključuju mali broj ispitanika, previše su heterogeni da bi se njihovi rezutati mogli meta-analizirati, koriste brojne surogatne mjere ishoda i nedovoljno pozornosti pridaju sigurnosti proizvoda. Iako većina zaključuje da beta-glukan ima povoljne učinke na zdravlje i bolest čovjeka u okviru analiziranih tema, ti dokazi su vrlo slabi i nepouzdani zbog metodoloških manjkavosti. The aim of this study was to conduct a summary of the results about efficacy and safety of oral and inhalation commercial products of beta-glucan. The systematic review of randomized controlled trials (RCTs) which analyze the effect of commercial beta- glucan products on human health was conducted. Databases MEDLINE and CENTRAL were searched. Two authors independently screened titles and abstracts obtained by searching literature, full texts of potential relevant studies and extracted data from the included studies. A risk of bias was evaluated using Cochrane risk of bias tool. The 28 randomized controlled trials, which analyze the effect of oral and inhalation commercial beta-glucan products on human health, were included. Part of the included studies were conducted on adults, and part on children. Effect of beta-glucan on various conditions related to health and disease was analyzed in those studies. Included studies had average number of 33 participants per group, high or unclear risk of bias of at least one domain, none of the studies had the conclusion including safety in the abstract and used large number of outcomes, many of which were surrogate. The evidence from RCTs about the effect of oral and inhalation commercial beta-glucan products was of poor quality, with small sample size, too heterogeneous to conduct a meta-analysis, using a lot of surrogate outcomes, and did not sufficiently address safety of the products. Although most of the included studies concluded that there is a beneficial effect of beta-glucan on human health and disease within the topics analyzed, this evidence is weak and unreliable due to methodological deficits. New large, high-quality randomized controlled trials are necessary to properly investigate the effect of beta-glucan on human health.
