Estes carcinomas estão associados a altas taxas de mortalidade, levando a óbito mais de 432 mil pacientes a cada ano, mundialmente. Uma vez que o diagnóstico precoce é essencial para melhor sobrevida dos pacientes, a identificação de biomarcadores circulantes e relacionados aos tumores provavelmente fornecerão informações úteis sobre o desenvolvimento da doença, a progressão e impacto na sobrevida. O papel dos microRNAs (miRNAs) na regulação de importantes processos celulares desde o desenvolvimento sugere sua aplicabilidade clínica como biomarcadores robustos. miRNAs são altamente estáveis e podem ser detectados nos fluídos corporais. A detecção de miRNAs no plasma e soro tem sido demonstrada útil no diagnóstico precoce e na predição de prognóstico de pacientes com neoplasias malignas. Portanto, nossos objetivos foram identificar o perfil global de expressão de miRNAs e os mRNAs-alvos regulados por miRNAs em tumor e plasma de pacientes com carcinomas de pâncreas e identificar vias moleculares reguladas por miRNAs. O miR-378h apresentou expressão significativamente aumentada (fold change ≥ 2; p < 0,05; FDR < 0,05) no tumor e no plasma de pacientes com adenocarcinoma ductal pancreático. Este miRNA atua como modulador de vias de crescimento celular e de resposta imune. Em adenocarcinoma de papila de Vater, o miR-5000-5p apresentou expressão significativamente diminuída (fold change ≥ 2; p < 0,05; FDR < 0,05) no tumor e no plasma, modulando vias envolvidas na sinalização Wnt e na proliferação celular. Este estudo contribuiu para a identificação de miRNAs circulantes e relacionados ao tumor como potenciais biomarcadores tumorais específicos importantes no processo de oncogênese pancreática. Pancreatic carcinomas are aggressive carcinomas and are among the tumors with high complexity at diagnosis, usually diagnosed in advanced stages. Pancreatic carcinomas are associated with high mortality rates, with more than 432 thousand deaths every year, worldwide. Since early disease diagnosis is essential for better patient survival, the identification of blood-based and tumor-related biomarkers will likely provide useful information on disease development, progression and impact on survival. Of note, the role of microRNAs (miRNAs) in regulating important cellular processes since early development suggests their clinical applicability as robust biomarkers. miRNAs are highly stable and can be detected in the body fluids. Detection of miRNAs in plasma and serum has been shown useful for early diagnosis and to predict prognosis of patients with cancer. Therefore, our aims were to identify the miRNA global expression profile in tumor and plasma from patients with pancreatic carcinomas and to elucidate molecular pathways regulated by miRNA target-genes. miR-378h was significantly upregulated (FC ≥ 2 and p < 0.05 and FDR < 0.05) in plasma and tumor from patients with PDAC. This miRNA acts as a modulator of cell growth and immune response pathways. In AMP cancers, miR-5000-5p was significantly downregulated (FC ≥ 2 and p < 0.05 and FDR < 0.05) in plasma and tumor, modulating pathways involved in Wnt signaling and cell proliferation. The identification of specific circulating plasma and tumor-related miRNAs deregulated in patients with PDAC and AMP may indicate these molecules as valuable biomarkers to assist in early diagnosis.
