Methods: This quasi-experimental study nested in a randomized clinical trial included a clinical sample of adults (18–60 years) diagnosed with major depressive disorder by the Mini International Neuropsychiatric Interview. The Millon Clinical Multiaxial Inventory III was used to assess the PDs. Defense mechanisms were analyzed using the Defense Style Questionnaire 40 and the intensity of depressive symptoms was measured by the Beck Depression Inventory. Data was analyzed using SPSS 22.0. The T-test was applied to compare the mechanism defense scores among depressed patients with and without Cluster B PDs. Pearson's correlation was used to evaluate the association between defense scores at baseline and depressive symptoms after intervention and at six-month follow-up. The adjusted analysis was performed by linear regression. Results: The final sample consisted of 177 patients diagnosed with MDD, and among these, 39.5% had Cluster B PDs. The mean score of immature defenses at baseline among patients with PDs was significantly higher when compared to individuals without PDs (p = 0.003). The results showed that, in patients with Cluster B PDs, the immature defenses at baseline predicted the persistence of depressive symptoms after intervention (adj. r2 = 0.501; β = 0.530; p
